fulfilling career with us! Key Responsibilities: Deliver
exceptional customer service to residents, their families,
responsible parties, and visitors. Coordinate and provide nursing
care to residents within the LPN scope, including catheter care,
wound management, dressing changes, nebulizer treatments, ostomy
care, and medical equipment management; oversee medication
administration, including oral medications, injections, and PRN
medications. Supervise the care provided by Home Health Aides
(HHAs) and Resident Care Assistants (RCAs). Manage and administer
medications while assisting residents with self-administration.
Foster positive collaborative relationships with residents,
families, and healthcare partners. Report any significant incidents
or changes in residents' conditions to Nursing Leadership (Charge
RN), Primary Care Providers, and families as necessary. Familiarize
yourself with each assigned resident’s Individualized Service Plan.
Assist in the training and orientation of Resident Care staff.
Document care and services provided to residents in the electronic
health record, adhering to nursing best practices and documentation
standards. Comply with all organizational policies and procedures
as outlined in the manual. Attend and engage in scheduled
in-service programs, training sessions, and staff meetings. Help
with the scheduling and staffing of Resident Care personnel as
needed. Provide coaching and mentorship to members of the Resident
Care Team as required. Adhere to emergency procedures. Understand
and follow all safety practices and procedures. Participate in
health, wellness, and lifestyle programs, assisting with all
aspects of resident care and experience as needed. Perform
additional duties as assigned.
Education/Experience/Licensure/Certification: Must hold a valid
Licensed Practical Nurse license in the state of New York with one
of the following qualifications: An Associate's degree and a
minimum of two (2) years of full-time clinical experience in
nursing, with at least one (1) year in a home health care agency,
hospital, skilled nursing facility, or community health program
that involved caring for the sick at home. A diploma or Associate's
degree in nursing and at least four (4) years of full-time clinical
experience in nursing within the last ten (10) years, with at least
one (1) year in a home health care agency, hospital, skilled
nursing facility, or community health program that involved caring
for the sick at home. Basic first aid and CPR certification.
Preferred experience in assisted living, post-acute, or long-term
care settings. Experience with and/or a special interest in working
