Albany Medical Center seeks a dedicated Clinical Nurse III Post
Transplant Coordinator to join our innovative healthcare team. This
role focuses on the care and coordination of patients
post-transplant, ensuring optimal recovery and health maintenance
in a collaborative, multidisciplinary environment. You will be
central to patient care management, treatment planning, and
follow-up processes, consistently providing support across the
entire transplantation journey. Coordinate patient care before and
after transplant procedures. Monitor patient recovery and implement
care plans in conjunction with transplant surgeons and healthcare
teams. Educate patients and families on transplant processes,
post-operative care, and lifestyle adjustments. Maintain accurate
patient records and report on patient progress. Collaborate with
multidisciplinary teams to enhance patient outcomes. Engage in
continuous professional development related to transplantation and
patient care. Strong interpersonal and communication skills Deep
understanding of transplant care and procedures Ability to lead and
collaborate with a multidisciplinary team Expertise in patient
assessment and care planning Proficiency in medical software and
patient database management Medical Insurance Dental Insurance
Vision Insurance Current Registered Nurse (RN) license
Certification in Clinical Transplant Coordination (CCTC) preferred
CPR/BLS certified
