How You’ll Make an Impact - Create a positive, inclusive team
environment where clinicians feel supported and motivated. - Lead
and collaborate with an interdisciplinary care team to deliver
coordinated behavioral health services in the home. - Serve as a
trusted partner to clinicians, physicians, patients, and families —

clinical standards and timely patient care. - Oversee plans of care

outcomes. What We’re Looking For - Associate’s Degree in Nursing
(or related field) required - 2 years of home health or behavioral
health experience as a Registered Nurse - At least 1 year of
leadership or team support experience - Current state Registered
Nurse license - Comfort using Workday, HomeCare HomeBase, and MS
